Targeted Case Manager (TCM)

​PHG is looking to hire a full-time TCM who will perform intensive case management and community support duties for identified consumers that are at risk of being involuntarily hospitalized or have been discharged from a state facility or diversionary hospital. These duties may include: assessment, counseling, psycho education, monitoring, transportation, crisis intervention, and other support deemed appropriate on the individual’s treatment plan. Must be 21 or older with a Bachelor’s degree in a human ser4ice related field, have a valid driver’s license, and be able to pass a Background screening and drug test.
