Responsibilities include: Providing case management services to individuals who are pregnant or have delivered their baby and have a substance abuse disorder. Developing an individualized plan of care, performing care coordination, linking individuals to health and wellness programs, and utilizing Peer Recovery Support Specialist and Behavior Health care. Individuals in the program will receive case management services monthly and continue for two (2) years from the date they enter the program. Required education: A minimum of a four-year degree (BA or BS) in a human service field with at least two years’ experience in care coordination and has completed all BMS required care coordination training.
