​Responsibilities include: Providing case management services to individuals who are pregnant or have delivered their baby and have a substance abuse disorder.  Developing an individualized plan of care, performing care coordination, linking individuals to health and wellness programs, and utilizing Peer Recovery Support Specialist and Behavior Health care.
