(formerly known as the Waiver Service Coordinator position), a full-time position in Hampshire County. Case manage a caseload of up to 30 children and adults with Intellectual and Developmental Disabilities. Responsibilities include conducting monthly home visits and bi-monthly workshop visits, facilitating team meetings, developing and maintaining person-centered treatment plans, and locating resources for additional services. Individual must be well-organized, eager to learn, and a team player. A Bachelor’s Degree in the Human Services field and a valid driver’s license are required. Experience with individuals with disabilities is preferred. The starting salary is based on the amount of relevant experience. There are additional options to earn extra pay on a semi-annual basis.
