Title XIX Case Manager

​(formerly known as the Waiver Service Coordinator position), a full-time position in Hampshire County.  Case manage a caseload of up to 30 children and adults with Intellectual and Developmental Disabilities.  Responsibilities include conducting monthly home visits and bi-monthly workshop visits, facilitating team meetings, developing and maintaining person-centered treatment plans, and locating resources for additional services. Individual must be well-organized, eager to learn, and a team player. A Bachelor’s Degree in the Human Services field and a valid driver’s license are required.  Experience with individuals with disabilities is preferred.
